Key Findings

  • Exercise therapy reduces pain and improves function in chronic low back pain
  • Individualized, supervised exercise programs are more effective than generic programs
  • Combining strengthening, stretching, and aerobic exercise yields best results
  • Effects are maintained at 12-month follow-up

This Cochrane Review analyzed 249 randomized controlled trials involving 24,438 participants to evaluate the effectiveness of exercise therapy for chronic low back pain.

The review found moderate-quality evidence that exercise therapy is more effective than no treatment or usual care for reducing pain and improving function in adults with chronic low back pain.

What Type of Exercise Works Best?

Individualized, supervised exercise programs showed better outcomes than generic, unsupervised programs. Programs combining multiple types of exercise (strengthening, stretching, and aerobic conditioning) were more effective than single-mode programs.

Clinical Recommendations

Patients with chronic low back pain should be prescribed an individualized, supervised exercise program that includes strengthening exercises for core and back muscles, flexibility training, and aerobic conditioning, delivered by a qualified physiotherapist.
